I copied boot.bin and u-boot.img to an SD card. It still does not boot, 
nothing is output on the UART either.

On flash, the most logical thing to do would be to put the "img" part right 
behind the SPL part. The linker can fill in the offset. I think that's what 
other SPL boots (like OMAP) do, it's been awhile since i hacked in those.
